Maybe I can chime something here ;-)

A camera that will take 5 images simultaneous and stitch in realtime does exist
to create a fully immersive video. Here is a link to the site.
http://www.ptgrey.com/products/ladybug2/index.asp Unfortunately it costs
$10,000.00 USD. :-(

There are other ways to create images from a normal camera for example check
out this website:
http://www.gigapan.org/ & http://www.gigapan.com/ - They built a robotic system
that uses a simple camera to create very big Panoramas. They don't accept
images under 50M pixels.
I don't know the price for this system.

I use a different system Canon 450D with a Fisheye lense and a Panorama head on
a tripod to create an Equirectangular image. I then use Pano2VR from Garden
Gnome http://www.pano2qtvr.com/
To extract the Cube faces from the Equirectangular images. It would cost you
approx. $5000.00 if you had to buy all the gear to get started. But you should
be able to do it for under $3000.00 if you buy cheaper equipment and software.

Ahh- I haven't heard of an app that does that with photos, but I am assuming
it would work the same way if you had took 6 pictures. There could be cameras
that do that (that have 6 lenses etc).

I think it can be done by hand but you would need to place your
tripod/camera perfectly, and even still do some photoshop adjusting...

So for a real life photograph (since it is so hard to recreate the precision
of the math in a 3d app), your best best would be something like the quicktime
VR layout, or other "mapping" of the skybox or "panorama".

Maybe google street view uses a system like the 6 images...but I am sure
those cameras cost a lot!!!! In general the cameras to produce "perfect" images
cost alot.

There are hacks to create panorama images with a standard camera (such as use
a shiny christmas ball). And there are software to create panoramas from many
images, but I don't know of any offhand that will create the 6 image layout for
you...if someone does please post!

So the 6 images seem like its best left for something super accurate like a
3d renderer.

Yup. It answers how to make this in a 3D application, but how would
one do this for a photographic texture?
Is there a software that can convert from Cubic Panorama to Skybox6
    format?
