Need a name, date of event (death or whatever) and location.

Death certificates varied over time.  Some ask certain questions, others
don't.  And your ancestor wasn't the person giving the info on his own
death certificate.  So many things on the death aren't always accurate.
But there may be clues you can get from the death that will lead you to
look at another source.

The port for Sao Miguel has records LEAVING the island.  They are online at
the CCA Web site (Centro de Conhecimento dos Acores = Center of Knowledge
of the Azores).

Look for the green words that say "Pesquisa de Passaportes."  Click that.
>From the drop down, click Ponta Delgada then click the Procurar button.
Chose the year span you want and then go page by page looking for him.

Since you didn't give any dates on your ancestors, I don't know what
censuses you checked.  Many of the U.S. Censuses ask for year of
immigration and if naturalized and what date.
