I'm sorry I haven't clearly explained that already.

Maria Leal da Rosa, daughter of Francisca dos Anjos, granddaughter of Joaquim José Leal da Rosa.

Manuel Joaquim Leal, son of Manuel Joaquim Leal da Rosa, grandson of Joaquim José Leal da Rosa.

They are 1st cousins.

The way I figure it is that my ancestor Manuel Joaquim Leal, who make his fortune in construction in Chile, moved back to the Azores and settled in Angra. You already know he was native of Piedade, Pico. He needed an operation for some reason and was rich enough to seek out the best. He ended up going to Lowell, Massachusetts and died from the operation. I believe he was only 40 or so, off the top of my head.

How your own line ended up there, I don't know why or when.
Maria Leal da Rosa was 18 years old when he died. She married 11 years later to António Silveira Pereira. How long had she already been living in New Bedford? If you haven't done so already, you should find her as a passenger arriving in Boston or New Bedford, most likely. It will shed some light on that. Maybe her parents were living there for a while and she just stayed. Maybe she traveled there with a relative. Maybe she and both siblings and parents all traveled there.

We don't yet know when or where her mother died, Francisca dos Anjos, except that he was after 16 Jan 1899 when José Leal da Rosa died. By the way, it might be worth trying to obtain a copy of his will mentioned in his óbito. Since he was living in Piedade in 1899, if he did go to New Bedford with his family, he later did return. Whether his wife stayed there or also returned, I don't yet know. She might have died many years later in Piedade, or maybe New Bedford.

In summary, this would be interesting to figure out for your own family history.

I can understand that for the Azores, because if some family doesn't pay for the upkeep, it will be used by anyone else.

My grandfather is buried in the Conceição, Angra cemetery and I don't remember now, but there are many others buried in that same spot. I think some are related, but not sure if they all are.

I don't know this João J. Pacheco, either.
Since the first person in the grave is my ancestor - the same one who married my ancestor from Chile - I think those who followed in being buried there are more likely friends or in-laws of your ancestor's generation.
