Apparently this gives a lot of confusion.
In this instance, the groom was a widower and is said to be related in the 1st degree of afinidade.
To understand that, one must realize being related in the 1st degree is like two siblings. So of course, these people are not siblings. What they must be are siblings by marriage. The groom is marrying his sister-in-law.
His two wives are the siblings by consanguinidade.
It happens to be almost the exact scenario of my latest article that came out this week in O Progresso.

The impediment is first degree affinity. Maybe she was his daughter-in-law or step-Daughter.
