Thanks for your response.  My wife just told me there is an expression that 
if someone "é de famílias" it mean they are from a family with prestige.  
Maybe this is why she has the "Dona" title whereas the mother and both 
grandmothers do not.  

Anyway, the priest did not name her family either inadvertently, or because 
the family was so well known he didn't think it necessary.
