He died at 62 years old (line 9: de idade de seissenta e dois annos) He was single (margin says "solteiro") He left no children (end of line 13 to line 14 "nao deixou filhos") He died at his mother's house in place in Cedros called Cascalho (starting midway on line 3: "em casa da sua mae no lugar do Cascalho deste freguesia dos Cedros, concelho da Horta, Diocese Angrense") It says he died having received all the sacraments of "Santa Madre Igreja" (Holy Mother Church) which I believe is the phrase you are referring to.
