The church has always maintained that any Christian may baptize an  individual 
who is in danger of dying before he can be baptized by a priest or deacon. The 
formula must be exact "I baptize you in the name of the Father, and of the Son 
and of the Holy Spirit." Many babies were baptized at birth, usually by the 
"parteira" (the midwife) or some member of the family. Those baptisms were 
usually recorded by the priest in the parish registry. A sub condicione baptism 
is when the priest then re-baptizes the baby in case the original baptism 
performed at home was not valid for whatever reason (usually because the 
correct formula was not used).

My understanding is that a baptism may be recorded "sub condicione" by a priest 
when it is the second baptism of an individual, and the priest has a concern 
that the first baptism might not have been performed exactly right-- such as a 
baptism done by a lay person.  The term describes the condition of the 
baptismal rite rather than that of the baby.
