I should add one more thing, which may be why devs haven't been able to 
repeat the problem:

The problem _only_ occurs for tapes which have been put into the pool 
using the "add" command and which are in the changer (tapes "add"ed but 
not physically present are generally ignored) - I rely on Bacula to add 
labels when it notes these new tapes are blank on first loading.

These tapes sit in the scratch pool in "append" state, no lastwritten.

Older tapes (purged, used, recycle, etc) are never pulled out of the 
scratch pool.

I got one private mail about older bugs, but they're not quite the same 
as I'm seeing.
