Personally I'd prefer defaulting this to something like <#todo: describe this image#> I would see this as a kind reminder to add a proper description. If I would like to have an empty alt, I'd had to do this deliberately and it would be only a tab-delete away. The images I'm inserting almost never have human written fields and I doubt that there is a good solution for auto-filling that attribute that pleases everybody.
