A quick update on this - my language for this file was set to "HTML"
as I'm often editing HTML+PHP files, and generally want the
highlighting of both.

When I switch the language back to PHP, the code highlighting is
applied correctly. So it's only when the language is set to HTML that
the php code folding breaks eventually.

So switching to php is fine in this case as a workaround - but I'm
still curious to know why the highlighting is limited when using the
"HTML" language..
