Is it just one specific instance of Outlook or would it still work if
the user logs on and starts Outlook on a different computer?

I would almost willing to bet that it is desktop redirector...

Please try the following to check:

1. Have user close all Outlook instances 2. Find user on the BES and try
sending 
        (a) IT Policy to the device. - does it gets to the device?
        (b) PIN message to the device. - does it get to the device?
        (c) Email message (from the BES).

If (a) and (b) work while (c) does not, it is almost certain that the
redirection is set-up for the desktop and not server.
