It sounds like BES can't communicate with the device. Have you verified that 
your connection to RIM is up?

Is this BES 5?  Is it a new install or conversion from BES to BES express. If 
BES 5, the sync service is usually set to manual as it is controlled by the 
controller service for HA on start and stop. Did you install as a standalone or 
primary for HA pair? 

You typically don't see the ETP.dat message as it will disappear when BES see 
it and re-routes back with verification to the device.  Make sure anti virus or 
spam or mail rules is not blocking or redirecting the ETP.dat. 

What is the device showing? Error? Or Timeout? 

Also, Is this account provisioned correctly for enterprise through the carrier 
and the account enabled for external services for BES?

Was the device used prior to activation on another BES and was it  wiped prior 
to activation?

Does BES have proper access to your mail environment and mail files?

Activations can break in many places.
