Unzip the tarball in /usr/share/squeezecenter instead of
/usr/local/slimserver. Make sure everything is recursively chown
squeezecenter and that Bin/mplayer.sh is executable by that user.

This looks like you also have a mysql problem too. Did your scan
really complete successfully?
