I really like Twonky. The UI in the browser is pretty minimal (although there is now a flash version). But most of the time you view the content on a DLNA compliant device, so you use that interface. I only use the server version, but they also have some other products for media management. Never used them though.
Twonky support custom browse trees, which is great. And it runs on a lot of different hardware and distributions. Hardware requirements are very minimal. It ran great on a NSLU2 with a 233MHz CPU and 32MB RAM.
