At long last I'm seeing this too!
I finally took the step I always thought was irrelevant to understanding
what was going on: I deleted the library.db and started from scratch. A
simple wipe & rescan on an existing library.db did _not_ expose the
problem.
Now that I'm able to reproduce the issue I'll hopefully find what broke
the behaviour pretty quickly.
