Ah, I did miss-understand. Perhaps it would make more sense to users  
if there were a menu item to add a "Field Group." It seems a bit  
obscure functionality to have to add the groups by typing a new title  
into the Groups keyword in an individual reference. On the other  
hand, I guess that might increase confusion to the already large  
number of group types that can be added, but maybe that's more of a  
documentation issue.
