To follow up on this, asking NCBI about it is almost certainly a waste of time. Here's a note that I ran across last weekend while working with the XML format: http://www.nlm.nih.gov/pubs/techbull/nd07/nd07_diacritics.html So MEDLINE will always be 7-bit ASCII, and only the XML format will have the full information.
