I find that having lights at different levels is very effective. A steady beam at the handlebars and a white front/rear red blinkie on my helmet - something like this: http://thelazyrando.files.wordpress.com/2008/11/wb7.jpg?w=510&h=412. I noticed that after I started using a rear red blinkie on the helmet I got about a foot more clearance from cars passing me. If you get a tight beam for the front blinkie you can actually point it into a windshield to make sure they see you.
