Both of these scenarios would be "bicyclist error" or rudeness in my
opinion. My technique, which seems to both work well, and be appreciated
by other path users, is to
a. slow down,
b. give a verbal warning well in advance of approaching other users,
c. wait for the people in front to acknowledge my warning (looking over
shoulder, moving right, nodding, etc.)
d. pass at a reasonable speed when I am sure they know I'm there.
e. SAY THANK YOU.

I've seen more and more bicyclists fail to warn people.  In the last 5
weeks, 3 people have warned me they were passing (7 times total, one
guy passes me about the same time every, so I didn't always see him.

I always warn, even the peds who are obviously wearing headphones and
can't hear jack.  Pedestrians tend to wave or say "thanks", though it
often seems like other bicyclists say "thank you", or something that
sounds like it, at least.  I've only ever had one person actually step
into my path in 12 years of path riding.
