Check the grey box and rescan the catalog (select the line so it's highlighted, click the rescan (the circular arrow) button at the bottom of catalog panel).

I assume you're iPhoto library is in the standard place. I doubt QS can find it if it's not.

The Plug-ins item in the menubar is badly named. It's a place where plugins can install their own sub-menus. The only two things that ever appear here are the Clipboard and Shelf.
