It was 3.5.13. I upgraded to 3.5.15 after your email but still have the same issue.

Yes Safari is 5.0.6 and it does have extensions tab. Does it need to have any specific extension installed?

Am I the only one in this group still on Mac 10.5.8? :-(

More specifically - does 10.5.8 have a version of Safari that uses extensions? (If you go Safari>Preferences is there an extensions tab?)
