The one I've more or less got fixed is what you mention above. The "Start at login" checkbox in Quicksilver's preferences doesn't correctly add or remove it from your Login Items. The other problem (the one in this thread) occurs when you do have Quicksilver in your Login Items and you attempt to use it for the first time after logging in. And then there are people who see it twice in the Dock on login. So maybe three problems. Anyway, I don't have any ideas on the last two.
