I can't replicate either problem, but yes they might well be
related.

On an 8.1 system, with firefox at the bottom of the screen the
Open File dialog is slightly above firefox's window (still perfectly
visible, this is about 70% of the way up the screen).  But if I move
firefox to the top of the screen, Open File opens below the top of
firefox's window.

On a system from last week (57.0.1, but the changes from 57.0 are
minimal) - if I maximize firefox and then kill it, it initially
opens in its normal place and then moves to full-screen.  If I close
it regularly after maximizing, it opens full-screen when I next open
it, and the Open File window is approximately in the centre of the
screen.

After maximizing, the maximize icon doesn't do anything (I expected
it to restore to normal size and position), but I can grab the
corner of the window and pull it down and in to be my normal sort of
size.

I can't test this on my low height 1600x900 monitor, I've swapped the
machines over but at the moment I'm not getting a video signal from
the desktop box which is now sitting there (and it is mostly
retired, because of iffiness - I suspect the mobo undervoltages the
8GB of RAM).

My netbook (1024x600 screen, i686 atom) does run (binary) firefox
maximized, and when that box powers off while sleeping, which it has
been doing a lot in the last 2 weeks, firefox resumes at its
maximized size.

All my desktop machines at the moment use VGA monitors (15-pin
D-SUB) and are AMD (radeon, radeonsi) or intel.  What is your screen
size (pixels, actual) and how is it connected ?  Also, what brand of
video card/chipset ?  If you are using a High DPI screen, maybe item
2.1.1 in the Arch Wiki will help -
