miranda is really not that bad once you get it configured it's just a pain
in the ass to get going at first. I think I tried the accessible one a while
ago and had good luck with it as well but now am just using the knewest
updated version with the scripts and the dll that is needed. Only thing I
had trouble with is getting the sounds configured with the msn part of it
but that's just because I wasn't sure what sound needed to go where as there
are alot of different options for attaching sounds to. Once I got it going
though, I loved it.

I did such a turtorial a while back for the JAWS friendly Miranda about two
years ago, I believe.
