If you are talking about a concrete driveway which has been poured then having 
a machine in to lift it out is probably the best although it is not impossible 
to regrade the area and reuse the pads. The trouble is that the bottoms are 
likely uneven and of various thickness so getting them level can certainly be 
tough. If the blocks are manufactured and therefore consistent then repairing 
is a lot more practical.

Usually though concrete slabs, manufactured or otherwise crack and split and 
that just is trouble.

  Our driveway is so old and pitted, I think we'd have to tear the whole thing 
up and start over again. This thing has thigh high weeds coming out of it, 1 or 
even 2 foot gaps between the paving blocks, six to 12 inch levels between the 
blocks. I look at it and it seems almost hopeless to fix without, as I said, 
tearing the whole thing down and starting over. Or just leaving it as dirt.

  Well now not all of this is necessarily correct.

  This sort of paving works best where there is a lot of traffic because the 
rolling keeps it warm and moving. In colder climates variable contraction and 
expansion resulting from heat and cold or from variable ground moisture can 
cause small cracks. Weed and other seed, dust, all sorts of stuff get into 
those small cracks regardless of the sub-soil and even small trees can begin to 
grow in these cracks. The power of this vegetation is really quite remarkable 
and will begin expanding cracks or even breaking up the pavement.

  There is a limit to how large of a crack one can satisfactorily repair with 
cold roll or other products and some of the restriction will depend on the size 
and weight of the equipment one can bring to bear but it is not at all unusual 
for the home owner to back cut cracks in pavement to produce a reasonably clean 
edge, scrape out the crack to remove the vegetation and then to pack in one of 
any number of commercially available products intended for the purpose of 
patching paved driveways. Some stores have so much of the stuff that they 
actually sell it. It even comes with recommended instructions.

  For best outcomes one should get at the damage early to keep the damage 
minimal but these patches can last years. 

  I think I would be tempted to kill the vegetation with something like Roundup 
a couple of days or a week before cleaning and patching to be sure any long 
roots are dead before patching over and maybe trickling in a little oil below 
the patch to discourage new growth which is probably very ungreen and not 
legal, maybe after dark when the blind can often work with impunity.

  Ground down to the solid base and possibly completely dug up back filled in 
  then freshly compacted. In other words none of us own such massive tools as
