how those forms are related to the specific pages? if you have
multiple forms, i suppose you have multiple actions that perform
uploads, so is the action that specify the criterias, not the page. or
am i missing something?
and those differentiated actions could theoretically be used on any
page, so if you have upload1 and upload2 actions, and you have upload
rights to page test, you can use both actions, provided you have read
access to both. so the way to control access is to restrict the action
pages.
