Please note that there are no changes being made to cpp impl.
The bugs fixed above are not applicable (b, c, d), and the change in
functionality (a) does not have any consumers - hence not implemented.
[Not consuming the returned seq-id to publish response should not cause an
error imo - please correct me if I am wrong].
