Can we really do this? It seems like we have a problem, because we'll have a 
boost.xml that contains:
  &function-index; <!-- includes function-index.inc -->

However, we need to process boost.xml with an XSLT stylesheet to produce 
function-index.inc. Doesn't that mean that boost.xml will fail to parse until 
it's parsed?
