I have added a bunch of element definitions to the DTD. Those
definitions were extracted from the current definition, for example:
<!ELEMENT header ANY>
<!ATTLIST header
base CDATA #REQUIRED
name CDATA #REQUIRED>
<!ELEMENT namespace ANY>
<!ATTLIST namespace
name CDATA #REQUIRED>
All elements are defined to contain ANYthing and all attributes are
defined to be #REQUIRED. Obviously, that's not how it's meant to be.
So my proposal to proceed would be to start evaluating element by
element and to fill the definitions out the way you want them to be.
At the same time, we need to decide in which context to allow them in
the DocBook DTD.
While doing this, we'll most likely have to clean up a bit, but this
is probably a good thing anyway. :-)
