Hello Terri & co. In my humble opinion (and that of several of my colleagues (both sighted and blind), the Command Summary is structured correctly. It follows in a logical manner with each of the actions required e.g. obtain help for current operation, preceding the commands required, e.g. SPACE with H, in the case of the BT. As far as I can assess, the actions and their corresponding commands for Options Available at Any Point are all located in 19.1.3.
In terms of the overall structure of the Command Summary, it has been purposely set out so that it starts with sections covering the general or basic commands before moving on to more specific or complex commands. In regard to your issue with quotation marks, I understand that they are smart quotes and that I need to somehow turn them off in Word before creating the electronic manuals.
