The teflon hose can crack.

Mine had a leak in it from a crack that appeared to have occurred when
the case was put on.  The teflon hose between the boilers was
compressed against the edge of the rear boiler, and was leaking
slightly until the pump ran.  Then it was a squirt.

Do you have water under your machine?  if you do it may be the teflon
hose, if not it is more likely the vacuum breaker or p-stat.
