Is there anything else I should be considering as a likely root cause?

Well, it would seem at first glance that the problem you're having would be caused by to much pressure or a serious grinder problem. If you have a gauge that shows shot pressure and keep track of what it generally does than you should know if it's pressure. Also, if you're sure the grinder is not a problem than it's likely pressure and the OPV is the most likely culprit. The only other common failure items are group gaskets, the screen under the gasket and the pressurestat.
