In the world of medicine we measure clients treated, or units billed
(some people have short visits and some have longer visits), 
compared to man hours (sometimes those are adjusted to
some sort or equivalents to reflect salary differences in staff). 
Kind of an FTE equivalent like teachers in some ways, and
yes we keep getting lower pay from insurers (and calls to
do more with less like everyone else).
