I think this problem is fixed in ddrescue 1.17. You can find an explanation in this thread[1].

"A CD-R disc written in Track-At-Once mode has two unreadable run-out
blocks at the end of the track. Linux seems to shrink the reported
length of the track when reading the last sectors of a track fails. If
this happens, ddrescue cannot be restarted (it reports that the input
file is shorter than the log data)."

I feel so deeply indebted to all the people who've contributed to
ddrescue: I'm amazed that it's turned 3 illegible disks into a perfect
iso file, and I am a very grateful and happy man. Thank you again.
