Although this may confuse you more, the way I think of it is in Adobe
terms:

Outside a score, \paper does stuff like margins, and \layout does
things like \context. Except that \layout can also set the indent.
Inside a score, \layout still does things like \context, but it also
does margins.
There has been some discussion about rearranging these -- probably by
eliminating the \layout word. We can do somthing like \context{} to
specify those things (inside or outside a score), and we could do
\paper inside or outside a score. I'm waiting for Erik's work to be
finished before raising this issue again... and potentially also
waiting until 2.10 is released and we're working on 3.0 (ie breaking
backwards compatibility).
