Nothing, really.  These lookups don't get counted because nobody expected them 
to be so slow on Windows.

Even now I'm not absolutely sure they are the problem.  They are simply the 
only thing I can find that changed in the code, between .11 and .12, that could 
explain the difference between a tiny example that shows the problem and one 
that does not.
