Your question is good and I think that is what we are trying to resolve ...
keep in mind the X's that I used were "guesses" or what some of my states
were using. Once we get all the services added to the list we can work on
what most orgs using to bill on today and then review whether or not the IGs
are "designed" for that or does the service billing need to be adjusted
elsewhere.

Isn't the real issue with the type of IG to be used, the site of service?
Ruth's chart is really good but many of the services can be performed in
several locations. For example: a clinic service can be a free standing
clinic or a hospital based clinic. Any service that is hospital based and
contains technical services should be billed on the UB not the 1500. The
other issue is does the payer have the right to dictate which IG should be
used for a claim?
