Also, there are some items with more than one component.  For example,
the surgery billed by the hospital also has a component billed directly
by the surgeon on a 1500 claim.  And DME can be billed on a 1500 type
claim even when there is a prescription that needs to be sent as an
"attachment".

It gets rather tricky.
