You might try feeding it the identity affine transform, i.e.:

1 0 0 0
0 1 0 0
0 0 1 0
0 0 0 1

To be completely honest, not many people use the AFNI-caret linkage anymore. They either view volume(s) and surface together in Caret, or they view their Caret-generated surfaces with SUMA, whose AFNI integration is more seamless than Caret's. I don't consider this a problem as much as a consequence of how Caret and SUMA have evolved over the last couple of years. It's only a problem if users' needs don't get met by either package.

You know you can view your functional volume overlaid with your anatomy in Caret, right? You might have a look at the PALS tutorial, just so you can get up-to-speed with the latest enhancements. I know you've been using SureFit/Caret for a few years now, but much has changed in the last year or so.
