What type of machine are you using? CAS doesn't require a lot of RAM, but because of its reliance on SSL it can require a relatively powerful CPU (we've used either a single CPU machine and dedicated SSL Accelerator or a dual-CPU machine without issues).

Slow ramp ups are ok, but on concurrent login spikes it die with 500 errors on ticket validation. However, before I start to panic I thought I ask if anyone else has done load or stress testing. What sort of results were you finding. I've not run a profiler on my code either...I'll be doing that today. I also have to deal with configuration of a jboss cluster, a Big IP, to mention a few things.
