Fuel filters have been replaced religiously....could be dirty fuel I guess....but it's a problem that has been gradually worsening over time. I rarely have the need to exceed 2200RPM so it's not been a problem I've been anxious to address. I probably try removing and inspecting/cleaning them.

Have you replaced fuel filters lately? They can get clogged up and reduce rpms. Also, if fuel is old it can result in increased sooty emission. Or it may be as you suggest, bad injectors. There is probably a product you can run through the system to try to clean them out. I'd try all of the above before replacing injectors.
