Really you have two choices:
1) Plug the card directly in the cable. You get most of the basic cable
channels and local channels (at least those delivered in analog) and you can
watch one channel on the computer while watching something else on the
"main" TV.
2) Plug the card into a feed from the cable box. You now get ALL the
channels, but must watch the same thing on both the computer and the TV.
You're third choice would be to get a second cable box dedicated to the
computer.
In the end this really no different at all than any VCR or PVR - you have
the exact same choice to make there. Everything (tuner cards, PVRs, VCRs,
etc) comes with tuners but ONLY the cable box tuners can decode many of the
channels.
Now things might be getting better. Comcast, for one, has announced that
their second-generation PVR boxes (we've been told due out by the end of the
year) are dual tuner boxes. Now they've not announced what they'll be
supported but in addition to allowing you record one thing while watching
another (something you just can't do easily with premium channels as you've
found) it should also allow for dual outputs.
This (with luck) should mean that I can FINALLY take advantage of my TVs
picture in picture and allow people to set up one feed to the computer and
the other the TV.
But we'll see what they decide to support.
