Typically, the new host will make a request to take over the domain. 
the admin contact will be emailed a confirmation letter, usually with a 
link to let the change happen.  Often, this requires re-registering the 
domain for another year, but that's usually not a big deal.  We have 
some registered through like 2013, though.  :-)

Anyway, this is the client-is-minimally-involved method.  It's usually 
what we have to do, since the clients don't know how to transfer it 
themselves.

One note:  I don't know that much about this.  I've only been involved 
with the problem children, so there may be a much easier route to take.
