I've lost shipmates myself.  I had to bury a roommate when he went
white-water rafting.  The raft he was in hit a fluke and he knocked out of
the raft and unconscious.  His life vest caught on a tree in the water and
the current pulled him under.  He was pinned and drowned.  The hardest thing
I had to do was escorting his casket home and meeting his family.  Even now,
almost 5 years later, it still chokes me up.

Accidents do happen, that is because no individual is perfect and no gear is
perfect.  I was a tech in the Navy, and my job was to make sure the gear
that I dealt with ran within operational specifications.  They weren't
"perfect", there were tolerances for everything.  And gear does break.
