You should probably actually inquire about a tellecommute option
before telling them anything.  Nothing's more random and annoying than
an employee giving notice without allowing you to correct a problem. 
You don't have to tell them you are leaving, just tell them your
problem and ask if there is anything they are willing to do to help
you out.
