This reminds me of a friend of a friend story.

My friend works on internal servers at the Folsom, CA Intel campus.  The
servers are rated by $/time that they cost the company when down.  So
the $100k/minute server receives attention before the $10K/day server
when things go bad.

One day something bad happened that completely cleaned out one of the
100k/min boxes.  No problem, it is backed up to tape every night by a
department solely in charge of backing up data for the entire company.
The get the latest tape, completely blank as was every other tape in the
library.

The night slave was just pulling a tape from the drive and filing on the
shelf for months with no concept that nothing had actually ever been
written to one.
