The entire argument is convoluted logic and meaningless for 2 reasons:

(1.) There will be a limited number of machines no matter the system
or country.  Why?  Because who's going to fund 10,000 cardiac machines
if the mean use is 3 per year?

(2.) If America didn't exist, then Canada would have to plug the
coverage hole, but it does so why should they?  That's like saying
that the convenience store concept has failed because you can find a
larger selection at a grocery store.

As for surgeries in India, it ain't just going to be elective
surgeries.  It's going to open-heart surgery, whatever.
