The one I heard about (a friend of a friend story) was a high speed paper
feeder.  (My father's a machinist specializing in printing equipment and I
worked building maintenance several summers.)

The paper feeders take rolls of paper averaging about 2 tons.  They run at
something like 200 feet per second.

Clamp trucks bring the rolls in and hang them on hydraulic racks.  Workers
(splicers) connect the rolls of paper together on the fly and at speed.  The
machines we had had an audible alarm if they "saw" something past a certain
point then shut down automatically if it went further.

When we work near them you hear all sorts of stories.  Guys getting hanged
or decapitated by jewelry or neckties, fingers and gloves getting crushed or
ripped off, etc.

Of course you never SEE this kind of thing happen... but everybody seems to
have a story.  ;^)
