Oh, and tie the premiums of the catastrophic $50,000 deductible plan
to my lifestyle based on certain metrics:

* physical fitness,
* blood lipid profile,
* other blood test factors

Also give me rewards for action: receipts for organic super foods,
getting a yearly physical, taking any medications, et al.

And, oh, make healthy foods count as a "health cost" and available for
use from my HSA.

That would make my premiums like $1/month.
