I favor a government financier in addition to private models to start
with. The market has been warped enough that the classical small scale
sticks and carrots seem to have been utterly ineffective over the
course of decades. I'd wished it were otherwise and that a pure
private market solution with government regulation would do the trick.
But we've been trying that for a long long time and it has been a
notable colossal failure.

That is just the beginning though. That takes care of some short term
issues and helps both reframe the debate as well as deal with short
term cost control. However, it does not address long term cost
control, which is going to have to take on the current
pay-per-procedure paradigm. Attacking health insurance reform is a
first, necessary, step. All the stuff we need to tackle after that
will have a substantial influence on what health care looks like in 30
years, so I can't say that a public option is anything more than a
good idea for the next 10 years while we figure out everything else.
It very well might go away. It will change. How? A fine question and
one to take a look at on many more discussions.
